Student Accident also covers Volunteers

Our Volunteer (& Student) Insurance is provided by BMI Benefits.

Instructions for making a claim:

1. The parish, school or agency completes the Student Accident Form Part 1-A and gives it to the student's family.
2. The Volunteer, or if a student, (the parent or guardian) completes the balance of the form.
3. When the form is completed, they submit it directly to BMI Benefits at the address above.
