This memorandum contains important information about your benefits and the upcoming Open Enrollment period that will run from Monday, November 2 through Sunday, November 15.
BENEFITS INFORMATION SESSIONS SCHEDULED: This year's open enrollment process will be very different than in past years. We will be using a new benefits system and introducing some new benefit offerings. In order to be sure that everyone's information is correct in the system and everyone has an opportunity to enroll in any of the new benefits, we are asking that all active priests and full-time employees participate in the open enrollment process this year. All information sessions will run from about 3:30 to 5:00 PM.
PLEASE PLAN ON ATTENDING ONE OF THE FOLLOWING INFORMATION SESSIONS:
Representatives from our benefit providers will be available with information and to answer your questions before and after the formal benefits presentation.
WELLNESS 2015 - NEXT STEP: COMPLETE YOUR HEALTH RISK ASSESSMENT (HRA) BY FRIDAY, OCTOBER 23: Congratulations to approximately 1100 of you who have completed your biometric screenings. You are halfway to earning a $600 discount on your medical plan premiums for next year. Results started being released this week and everyone's results should be available by next Friday, October 9. If you provided a valid email address when you registered at the Wellness Fair, you should receive an email from [email protected]. If you did not provide an email address, or do not receive the email, you can still check on your results and complete the HRA by going to www.myhealthcheck360.com and Clicking the blue LOGIN button to register. (If you would like to complete the HRA in Spanish, once you register, you may select the Spanish option in the upper right corner of the screen.) DEADLINE FOR COMPLETING THE HRA IS FRIDAY, OCTOBER 23 OPEN ENROLLMENT PERIOD: NOVEMBER 2 - 15: This year's open enrollment process will be very different from prior years. We will be asking all Priests and full-time employees to participate (even if you are not enrolled in our benefits.) We need to check your personal information and to make sure your beneficiary and dependent information is accurate. If you are waiving our medical benefits, we need you to complete an online waiver. We will also be offering some new voluntary life insurance and other benefits through a company called Advanced Voluntary Concepts (AVC). Some of these products being offered to current employees must be selected now when there is no evidence of insurability required. If you wait, you may not be able to obtain the coverage in the future. AVC will be scheduling appointments with each of you at your work location, during the Open Enrollment period. More information about the Open Enrollment process will be shared at the information sessions and will be mailed to your home addresses during the last week in October.
